Service description:
This team works with children from birth to 19 years and has responsibility for care for children with additional and nursing needs and/or disabilities who attend the Cedars, Gibside and Dryden special schools. They carry out assessments relating to ‘Continuing Care’ health needs and services can be provided at home, school and short break facilities.
