Eligibility / referral criteria:
Eligibility Criteria: Available to anyone who has completed compulsory education and has a learning disability, physical disability, mental health or autistic condition who meet/or would meet some substantial areas of FACS criteria. They should also have either had a social care assessment or are in receipt of Gateshead Council ‘in house’ provider services.

This service aims to:
• Raise awareness of employment options to subsequently raise aspirations of people with disabilities and their families;
• Support young adults with disabilities and their families in experiencing a smooth transition from education to work;
• Secure jobs which provide a person with opportunities for valued contribution and jobs which are based on their individual interest, abilities and requirements;
• Provide supported employment for people who need substantial, on-going assistance to be successful at work
