| Service description: | Neuro Partners are specialists in delivering tailored care and support packages to individuals, particularly those with neurological conditions. A Person Centred Approach is used in order to design and deliver unique packages of support in order to meet an individual’s assessed needs, improve their quality of life, fulfil their ambitions and aspirations and promote autonomy. They are registered with and approved by the Care Quality Commission.
The service specialises in supporting people with a wide range of conditions including Muscular Dystrophy, Learning Disabilities, Autism, Asperger’s Syndrome, Mental Health issues, spinal and brain injury, epilepsy, ME and complex needs.
A vast range of support services are available including tailored packages of care and support, complex needs services, community integration, domiciliary care, assessment and intervention planning, case management and co-ordination, occupational therapy support, specialist psychiatric nursing, physical and psychological therapy and intervention, brain injury rehabilitation, behavioural management and cognitive behavioural therapy.
