Service description: | Specialist Health Practitioner for children and young people with additional support needs
Description: This practitioner works with other colleagues to support families of children and young people with additional support needs, provides information and liaises with local agencies and services.
Some of the support that can be provided by the specialist health practitioner includes: • Support during and after your child’s diagnosis • Attendance at your child’s assessment and helping you with issues related to your child’s additional needs such as sleep • Working with you, health, education and other relevant agencies to help your child achieve the best outcomes possible • Liaison with you and services through Early Support, Common Assessment Framework (CAF), and panels • Attending steering groups, forums and panels that focus on children with additional support needs • Delivering training sessions for various professionals, families and establishments • Leading on the process of sharing results of new born screening results • Leading on the delivery of the Care of Next Infant (CONI) programme for families who have had a previous sudden infant death
